| 中文摘要:目的观察碳酸氢钠片联合塞来昔布预防伊立替康所致腹泻的疗效。方法回顾性分析了2009年7月~2010年3月笔者科室经治的接受含伊立替康方案化疗并采用碳酸氢钠片联合塞来昔布预防腹泻方案的晚期结直肠癌患者病例资料，比较腹泻及其他不良反应的差异，其中预防组（n=14），化疗前1天开始口服碳酸氢钠和塞来昔布（碳酸氢钠片0.5g，每天3次；塞来昔布200mg，每天1次），连用5天；对照组（n=21），无特殊预防用药。两组在化疗期间均常规用托烷司琼等止吐，化疗后如出现腹泻按规定给予盐酸洛哌丁胺及蒙脱石散剂治疗，必要时给予生长抑素及静脉补液支持。每周期化疗过程中均予以监测不良反应的发生及程度。结果给予碳酸氢钠联合塞来昔布显著降低了腹泻的发生率（预防组9.33%，对照组27.54%，P＜0005），同时也减少了骨髓抑制等其他不良反应的发生，差异有统计学意义。结论碳酸氢钠片联合塞来昔布胶囊口服能安全有效地预防伊立替康相关腹泻，同时也减少了骨髓抑制等其他不良反应的发生。 |

| Clinical Observation of Sodium Bicarbonate Combined with Celecoxib for Prevention of Irinotecan Diarrhea Induced by Irinotecan |

| Abstract:ObjectiveTo observe the therapeutic effects of sodium bicarbonate combined with celecoxib for irinotecan-induced diarrhea.MethodsA total of 35 patients with advanced colorectal cancers were allotted into prevention group (n=14) and control group (n=21) from July 2009 to March 2010 in our hospital. All patients received the chemotherapy including irinotecan. The prevention group received sodium bicarbonate combined with celecoxib before the use of irinotecan for 4d, while the control group received routine treatment. All adverse effects were evaluated using NCI-CTC standard.ResultsThe use of sodium bicarbonate combined with celecoxib reduced the incidence of irinotecan-induced diarrhea significantly (prevention group 9.33%; control group 27.54%;P＜0.005 ). Besides the diarrhea, the preventive therapy could reduce the incidence of bone marrow suppression, nausea and vomiting.ConclusionSodium bicarbonate combined with celecoxib was proved to be effective for irinotecan-induced diarrhea. |
