脑白质空泡样变的病因研究

中文摘要:目的用双环己酮草酰二腙(cuprizone)、地西泮、苯巴比妥诱导大鼠脑白质脱髓鞘、空泡样变并探寻其病因,证实地西泮、苯巴比妥未引起脑白质空泡样变。方法用环己酮草酰二腙制备大鼠脑白质脱髓鞘及空泡样变模型(酮腙组),与地西泮(地西泮组)、苯巴比妥(苯巴比妥组)、生理盐水对照组(对照组)比较,应用HE、MBP免疫组化及TUNEL凋亡染色,观察各组2、4、6周时脑组织结构变化及细胞凋亡。结果地西泮组、苯巴比妥组、酮腙2、4周组及对照组未观察到白质疏松及空泡改变,酮腙6周组可见脑白质疏松、髓鞘裂解、空泡形成,以脑干和枕顶叶最明显;酮腙4、6周组可见皮质下白质、胼胝体、脑干及小脑白质TUNEL阳性染色,与地西泮组、苯巴比妥组、对照组及酮腙2周组比较差异具有统计学意义(P<0.05);酮腙6周组TUNEL阳性染色明显多于4周组,差异具有统计学意义(P<0.05)。结论环己酮草酰二腙可诱导大鼠脑白质脱髓鞘、空泡样变,呈时间剂量相关性,随着时间延长和累积剂量增加,细胞凋亡及空泡形成增多;脑白质细胞凋亡早于空泡样变,提示胶质细胞凋亡可能是脑白质脱髓鞘及空泡样变的原因之一;地西泮、苯巴比妥未引起脑白质细胞凋亡及空泡样变。

Study on Etiologies of the Brain White Matter Vacuolation
Abstract:ObjectiveTo investigate the white matter demyelination and vacuolation in the rat brain induced by the cuprizone. MethodsCuprizone was used to make the rat brain white matter demyelination and vacuolation model(Cu). The structural change and cell apoptosis of the brain in each group were detected by HE,MBP immunohistochemisty and TUNEL apoptosis staining at 2nd, 4th and 6th weekend after treatments. ResultsThe white matter demyelination and vacuolation were seen in the rat brain induced by the cuprizone. Vacuolation was not observed in the brain of the diazepam in phenobarbital Cu 2 weeks and Cu 4 weeks groups. Widespread demyelination and vacuolation was seen in the subcortex white matter in the Cu 6 weeks group,with most severely in the brain stem and the the occipital parietal lobe white matter. Apoptosis was observed in the brain white matter in the diazepam, Phenobarbital and Cu 2 weeks groups.The apoptosis levels were significantly different in the subcortex white matter among Cu 2, 4 and 6 weeks groups (P<0.05). It was significantly higher in Cu 6 weeks group than in Cu 4 weeks group. ConclusionWhite matter demyelination and vacuolation can be induced in the rat brain by feeding the cuprizone. It was related to the dosage and time. Along with the time extension and the cuprizone dosage increasement, the cell apoptosis increased and the demyelination aggravated. The gliocyte apoptosis probably is one of the reasons of the white matter demyelination and vacuolation. Vacuolation was not observed in the brain of the diazepam, Phenobarbital and Cu feeded for 2 or 4 Weeks groups.
