胰腺实性-假乳头状瘤的多排螺旋CT表现与病理对照研究

中文摘要:目的回顾性分析与总结胰腺实性-假乳头状瘤(SPTP)的CT特点与病理对照,提高其术前诊断的准确率。方法搜集9例经CT检查手术病理证实的SPTP完整资料,5例行平扫+静脉期扫描,4例行动脉期+静脉期扫描。结果9例患者均为女性。胰体尾部6例,胰头颈3例,肿块直径2.7~12.8cm,平均8.2cm。囊实性混合性肿块9例,其中以实性肿块为主2例,囊性为主3例,实性与囊性成分相当者4例。平扫囊实性混合性肿块的实性部分密度较均匀,增强后轻中度均匀强化,动脉期轻度强化,静脉期逐渐强化,强化不及胰腺。囊性部分为无强化的液性低密度伴边缘有壁结节、乳头状、飘絮样或分隔状强化,包膜显示清楚,厚薄较均匀,轻度强化。胰管、胆总管均无扩张。结论多排螺旋CT的检查对SPTP具有定位、定性价值,为外科手术方案提供参考。

The Performance of Multi-slice Spiral CT Findings and Pathologic Correlation in Solid-pseudopapillary Tumor of Pancreas
Abstract:ObjectiveTo improve the accuracy of preoperative diagnosis in patients with solid-pseuopapillary tumor of pancreas (SPTP) through retrospectively analyzing the CT findings compared with pathological results. MethodsThe study included nine patients with SPTP who were surgically resected and pathologically proved. All patients were females and underwent contrast enhanced multi-slice spiral CT examination before operation. In five cases, plain CT scan and venous phase after administration of contrast agent were performed, and the remaining four cases underwent dynamic CT imaging consisting of plain scan, arterial phase and venous phase. ResultsThe mass located at the body or tail of pancreas in six cases, and at the pancreatic head or neck in the other three cases. The size of the mass ranged from 2.7 to 12.8cm (mean 8.2cm) in diameter. All masses were presented with the characteristics of mixed density, of which two cases were mainly solid, three were cystic, and the others were almost equally solid and cystic. In the plain scan, the solid component of the mass manifested as a uniform density and showed mild or moderate homogeneous contrast enhancement. From arterial phase to venous phase, the solid part of mass presented a trend of gradual enhancement, but the density was less than that in the parenchyma of pancreas. In contrast, the cystic part, which showed the liquid density, had no contrast enhancement. The wall of the cystic mass was clearly revealed after contrast administration, and it showed uniformity of thickness and mild enhancement. In the edge area of the cystic mass, mural nodule, demarcation membrane, papillary or catkin like mild enhancement could also be discerned. The pancreatic duct and common bile duct were normal in all patients. ConclusionThe multi-slice spiral CT examination with contrast enhancement provids valuable information in the localization diagnosis and qualitative diagnosis of the SPTP, and also in the programming of the surgical operation of the SPTP.
