| 中文摘要:目的探讨宫颈意义不明确的不典型鳞状细胞（ASC-US）的临床病理意义。方法对335例宫颈液基细胞学检查结果为ASC-US的患者采用二代杂交捕获（HC-Ⅱ）法检测高危型HPV感染情况，并在阴道镜下行宫颈活检病理检查。结果335例ASC-US患者阴道镜下宫颈活检病理检查结果中CIN 61例，浸润癌1例。ASC-US主要分布在31～50 岁年龄段，高度病变也主要在此年龄段；＞30岁组和≤30岁组的宫颈活检病理学检查结果，在≥CINⅡ的病变比较中，有显著性差异。高危型HPV检查阳性率为60.0％(201/335)，高危型HPV阳性患者和阴性患者在≥CINⅠ和≥CINⅡ病变率的比较中，有非常显著性差异。结论对ASC-US患者的处理应给予足够的重视，尤其是＞30岁者处理需要更加积极。高危型HPV检测对ASC-US的患者可有效地进行分流监测，提高筛查效率，降低宫颈病变的漏诊率。 |

| Clinical and Pathological Analysis of Atypical Squamous Cells of Undetermined Significance in Cervix |

| Abstract:ObjectiveTo explore the clinical and pathological significance of ASC-US in Cervix.MethodsTotally 335 patients diagnosed as ASC-US by liquid-based cytology were detected to be high-risk HPV infection by hybrid captureⅡ (HC-II).The cervical pathologic diagnoses were obtained under colposcopy guided biopsy.ResultsAmong 335 patients of ASC-US with pathological diagnosis of cervical biopsy under colposcopy,61 patients were CIN,1 patient was infiltrating carcinoma. ASC-US were mainly distributed in 31 to 50 age group. High lesions were also mainly distributed in this age group.There was significant difference in ≥ CIN Ⅱ between > 30 age group and ≤ 30 age group with pathological diagnosis of cervical biopsy.The high-risk HPV positive rate was 60.0% (201/335).There were significant difference in the lesion rate of ≥CINⅠ and ≥CINⅡ between positive high-risk HPV patients and negative patients.ConclusionThe treatment of ASC-US patients should be given adequate attention,especially > 30 years old and they need to be given more active treatment. High-risk HPV detection in ASC-US patients can effectively shunt,monitor and improve the screening efficiency and reduce the misdiagnosis rate of cervical lesions. |
