| 中文摘要:目的 探讨社区获得性肺炎(CAP)患者的血清白蛋白(ALB)水平与其严重程度及预后的相关性。 方法 连续选择从2012年1月~2013年12月在嘉定区中心医院急诊科诊断为CAP的患者共256例,根据患者ALB水平分为高值组(≥30g/L,n=229)和低值组(<30g/L,n=27),比较两组患者的临床资料、实验值指标、严重程度、PSI分级及病死率之间的差异性;再依据不同严重程度(如是否重症、PSI评分)及不同预后,分别比较ALB异常率的差异;采用Spearman相关分析对ALB与PSI评分进行相关分析。 结果 ALB低值组的重症率(18.31% vs 7.57%,P=0.021)、病死率(60.00% vs 6.64%,P=0.000)及PSI Ⅳ~Ⅴ级比例(62.96% vs 24.02%,P<0.001)均显著高于ALB高值组。重症组中ALB异常率显著高于非重症组(18.3% vs 7.6%,P=0.021);死亡组中ALB异常率显著高于存活组(60.0% vs 6.6%,P=0.000);ALB异常率随着PSI级别的增高而增高(1.1%、9.7%、22.4%、40.0%,P=0.000);Spearman相关分析显示ALB与PSI评分呈显著负相关(r=-0.531,P=0.000)。 结论 社区获得性肺炎患者的血清白蛋白水平与其严重程度密切相关,对预后有重要的参考价值。 |

| Relationship of Serum Albumin with the Severity and Prognosis among Patients with Community Acquired Pneumonia |

| Abstract:Objective To explore the relationship between serum albumin (ALB) with the severity and prognosis in patients with community acquired pneumonia(CAP). Methods From Jan. 2012 to Dec. 2013, a total of 256 CAP patients in the Emergency Department of Jiading District Central Hospital were enrolled in the study. Based on the ALB level, the patients were divided into high-ALB group(≥30g/L,n=229)and low-ALB group(<30g/L,n=27). Differences in clinical data, laboratory indexes, severity, pneumonia severity index (PSI) score and mortality were compared between two groups. Then the difference of ALB abnormal base on severity(such as whether severe or PSI score) and different mortality was separately compared. The correlation between ALB level and PSI score was analyzed by Spearman correlation test. Results Low-ALB proportion of severe patients(18.31% vs 7.57%,P=0.021), the mortality rate(60.00% vs 6.64%,P=0.000) and the level of PSI Ⅳ-Ⅴ(62.96% vs 24.02%,P=0.000)were significant higher in High-ALB group. With the elevated level of PSI, the rate of abnormal ALB was raised(1.1%, 9.7%, 22.4%, 40.0%,P=0.000). And the abnormal ALB rate in death group was significant higher than that in survival group(60.0% vs 6.6%,P=0.000). PSI score was negative significantly correlated with ALB level(r=-0.531,P=0.000). Conclusion The level of ALB has a closely related with the severity in community acquired pneumonia patients that has important reference value to the prognosis in CAP. |
