低能量Q开关Nd:YAG1064nm激光治疗黄褐斑疗效与安全性的评价

中文摘要:目的 评价低能量Q开关Nd:YAG1064nm激光治疗黄褐斑的临床疗效和安全性。 方法 检索Cochrane Libraray、PubMed、EMbase、SinoMed、万方、维普及中国知网数据库中相关随机对照研究,按照Jadad评分表,逐一评价纳入研究质量,提取有效数据,采用RevMan 5.0对同质研究进行Meta分析。 结果 本研究共纳入8篇文献,其中包含109例患者,对低能量Q开关Nd:YAG1064 nm激光与对照组治疗的总有效率进行Meta分析,差异有统计学意义(OR=6.00,95%CI:4.02~8.94)。亚组分析中,与口服活血疏肝中药相比,差异有统计学意义(OR=4.19,95%CI:2.27~7.73),其余因存在临床异质性,均做描述性分析。 结论 低能量Q开关Nd:YAG1064nm激光能在一定程度上改善黄褐斑的临床症状,尤其是短期疗效,但仍存在炎症和色素沉着等不良反应,且可能复发。

A Review of Low-Fluence Q-Switched Neodymium-Doped Yttrium Aluminum Garnet (1064 nm) Laser for Melasma
Abstract:Objective To assess the effectiveness and safety of low-fluence 1064nm Q-switched Nd:YAG(QS-Nd:YAG)Laser for the treatment of melasma by Meta-analysis of the randomized controlled trials(RCTs). Methods Electronic searches of Cochrane Libraray,Pubmed, EMbase, SinoMed,Wan Fang Data,VIP,CNKI were performed for all publications of RCTs on the Treatment of melasma with low-fluence QS-Nd:YAG laser.Methodological quality assessment and data extraction assessment for methodological quality was perfomed using a modification of criteria in Jadad score scale.RevMan 5.0 was used for Meta-analysis. Results Eight papers of RCTs involving 109 patients were included.Results of Meta-analysis showed that there was significantly statistical difference in the effectiveness (OR=6.00,95%CI:4.02-8.94).In subgroup,there was significantly statistical difference in the effectiveness comparing to chinese herb(OR=4.19,95%CI:2.27-7.73).Others were descriptively analyzed. Conclusion The evidence showed that low-fluence QS-Nd:YAG laser in treating melasma could also improve climacteric symptoms,especial short period follow. But there were many side-effects,for example, inflammation,hyperpigmentation and recurrence.
