| 中文摘要:目的 探讨性别差异对TWA的影响,旨在提高临床工作者对TWA的诊断率。 方法 选择武汉大学人民医院2012年8月~2013年10月行平板运动试验(treadmill exercise test,TET)的患者140例,其中,男性62例,女性78例,患者年龄52.64±9.69岁。采用GE公司提供的Marquette T 波电交替(microvolt T wave alternans,MTWA)分析程序,进行标准时域运动试验,并对其进行焦虑自评量表(SAS)的问卷调查。最后对患者的性别、年龄、MTWA、TET等数据进行分析。 结果 不同性别间焦虑评分存在差异(T检验,P=0.008)。性别因素与TWA结果相关性(Logistic回归分析,P=0.824,OR=0.761,95%CI:0.068~8.500)不明显,运动平板试验结果与TWA结果相关性(Logistic回归分析,P=0.499,OR=0.492,95%CI:0.063~3.853)也不明显。 结论 性别对TWA结果影响不大,但是临床工作者不能忽视性别差异,因为可能是性别差异对TWA的不同影响相互作用,导致性别对TWA的影响不明显。因此医生在下临床诊断时还应将性别纳入考虑之列,并结合患者的临床实际,实事求是的给患者一个准确的诊断,以此发挥TWA预测心脏性猝死和恶性心律失常的重要作用。 |

| Influence of Gender Differences on TWA |

| Abstract:Objective To discuss the influence of gender differences on TWA to improve the right diagnosis rate of clinical workers on TWA. Methods Totally 140 cases of patients who took the test of treadmill exercise were selected as the subjects,including 62 cases of men and 78 cases of women from August 2012 to August 2013 at Renmin Hospital of Wuhan University. Then standard time domain movement exercise was carried out through Marquetee Microvolt T wave alternans (MTWA) analysis procedure provided by GE corporation.And the investigation of questionnaire of self-evaluation of anxiety scale(SAS) was take. Finally,each patient's gender,age,TET,MTWA and the score of SAS were analyzed. Results Anxiety score was different between different genders(t test,P=0.008).The correlation between gender and TWA was not obvious(Logistic regression analysis,P=0.824,OR=0.761,95%CI:0.068-8.500),and so was that between TET results and TWA(Logistic regression analysis,P=0.499,OR=0.492,95%CI: 0.063-3.853). Conclusion Gender difference has little influence on TWA,but the clinical workers can't do it.After all,gender difference's different role on TWA may lead to the fact that gender does't affect TWA.So when doctors make a right diagnosis for patients,they should take gender's influence into consideration and combine it with clinical situation.Only on that way can we let TWA play the significant role in predicting sudden cardiac death and malignant arrhythmia. |
