短暂性脑缺血发作患者颈总动脉内中膜厚度与颅外血管事件及脑卒中复发相关性研究

中文摘要:目的 探讨TIA患者颈总动脉内中膜厚度(CCA-IMT)与颅外血管事件和脑卒中复发的相关性。 方法 研究共纳入212名TIA患者,采集患者一般情况资料、临床特征和疾病情况,进行头部CT检查和颈动脉超声检测,分析CCA-IMT与TIA风险因素的相关性;对患者进行为期1年的随访观察,分析CCA-IMT与颅外血管事件以及脑卒中复发的预测效应。 结果 共发生24例颅外血管事件(18例冠心病和6例外周血管病)和16例脑卒中复发。CCA-IMT与TIA多种风险因素相关,男性年龄>60岁以及具有高血压、冠心病患病史的患者中CCA-IMT显著增加。生存分析结果CCA-IMT对颅外血管事件具有良好的预测效应,CCA-IMT值>0.939mm患者颅外血管事件风险显著增加(P=0.000)。 结论 CCA-IMT较高TIA 患者颅外血管事件风险增加,CCA-IMT对TIA患者颅外血管事件具有良好的预测作用,可作为临床上常用的预测手段。

Correlation Study Between Carotid Artery Intima-media Thickness and Extracranial Vascular Event or Stroke Recurrent
Abstract:Objective To investigate the relationship between common carotid artery intima-media thickness and extracranial vascular event or stroke recurrence. Methods Totally 212 TIA patients were recruited in this study. The basic characteristics, clinical, neuroimaging, ultrasonographic, and etiological data were collected. Patients were followed prospectively for 1 year. Extracranial vascular events and stroke recurrence were recorded. Results Twenty four extracranial vascular events (18 coronary heart disease and 6 peripheral arterial diseases) and 16 recurrent strokes occurred during the follow-up period. CCA-IMT was associated with risk factors of TIA, CCA-IMT was significantly higher in population with some features,including male, age>60, history of hypertension and coronary heart disease. Survival analysis showed that CCA-IMT >0.939 mm as independent predictors of extracranial vascular events after transient ischemic attack (P<0.001). Conclusion An elevated CCA-IMT value was associated with a higher long term risk of extracranial vascular events. CCA-IMT could use as a clinical predictors for extracranial vascular events in TIA patients.
