彩色脉搏波新技术评价2型糖尿病患者早期颈动脉功能变化的初步研究

中文摘要:目的 应用彩色脉搏波技术(ultrafast pulse wave velocity,UFPWV)定量评价2型糖尿病(type 2 diabetes mellitus,T2DM)患者早期颈动脉功能的变化。方法 对55例2型糖尿病患者及55例健康对照者行颈动脉内中膜厚度(intima-medium thickness,IMT)测量及彩色脉搏波技术检测,采集脉搏波速度(pulse wave velocity,PWV),包括收缩期开始时PWV(beginning of the systole, BS)和收缩期末时PWV(ending of the systole, ES),分析各参数在两组间的差异性及BS、ES与IMT、年龄的相关性。结果 2型糖尿病组IMT、BS、ES值均大于健康对照组,差异有统计学意义(P=0.000)。BS、ES与IMT呈正相关(r分别为0.399、0.396),差异有统计学意义(P<0.05)。BS、ES与年龄呈正相关(r分别为0.357、0.604),差异有统计学意义(P<0.05)。结论 彩色脉搏波技术可快速采集、自动测量颈动脉PWV值,评估颈动脉管壁弹性功能改变。2型糖尿病患者颈动脉在结构改变之前已出现弹性功能改变,该技术可较早发现这一变化。

Preliminary Study of UFPWV Evaluation of Early Carotid Artery Function Changes in Type 2 Diabetes Mellitus.
Abstract:Objective To quantitatively assess the carotid artery function changes by using UFPWV in type 2 diabetes mellitus. Methods The measurement of intima-medium thickness(IMT)and UFPWV of carotid artery were performed in 55 patients with type 2 diabetes and 55 healthy control subjects. Pulse wave velocity(PWV), including the PWV at the beginning of the systole(BS) and the PWV at the ending of the systole(ES) were captured. The difference of parameters between the two groups and the relevance of BS,ES and IMT,age were analyzed. Results IMT,BS,ES in diabetic group were obviously higher than those in healthy control group,the difference was significant(P=0.000). BS and ES were positively correlated with IMT(the coefficient was 0.399 and 0.396,respectively), and the difference was significant(P<0.05). Bs and ES were positively correlated with age(the coefficient is 0.357 and 0.604,respectively),and the difference was significant(P<0.05). Conclusion UFPWV can quickly capture,automatically measure the carotid artery PWV,evaluate the elastic change of carotid artery. UFPWV can earlier find that there has been a change in elastic function before the change in the structure of carotid artery in type 2 diabetes mellitus.
