血清Cys C、β2 -MG在感染性休克早期集束化治疗中的临床研究

中文摘要:目的探讨血清Cys C、β2-MG在感染性休克早期集束化治疗中的临床应用。方法随机选取成人综合ICU感染性休克患者60例。予以早期集束化治疗。于集束化治疗前、治疗后6h、入ICU1、2、3、4、5天分别检测血清Cys C、β2-微球蛋白(β2-MG)浓度并分别记录各时间点APACHEⅡ评分。根据患者28天转归分为存活组与死亡组,两组不同时间点相关指标进行比较。本组内各时间点相关指标与集束化治疗前进行比较。分别将两组内血清Cys C、β2-MG浓度与APACHEⅡ评分进行相关分析。结果存活组APACHEⅡ评分血清Cys C、β2-MG随着病情好转下降,死亡组则呈升高趋势。经Pearson相关分析, APACHEⅡ评分与血清Cys C、β2-MG浓度呈正相关。结论血清Cys C、β2-MG浓度可作为感染性休克治疗效果及预后的评价指标,与APACHEⅡ评分结合,能更准确地评估感染性休克病情的严重程度及预后。

Clinical Study of Cys C and β2-MG in the Early Bundle Treatment for Septic Shock.
Abstract:Objective To study the cilinical application of Cys C and β2-MG in the early bundle treatment for septic shock. Methods Sixty cases treated by early bundle were selected randomly.The Cys C and β2-MG concentration were respectively tested before bundle treatment and after 6 hours and after entering ICU the first day,the second day,the third day,the fourth day,the fifth day.The APACHE Ⅱ score were recorded respectively in different time point. There were divided into survival group and death group according to 28d outcome.The related indexs in different time point between the two groups were compared and also the indexs within the groups were compared with the indexs before bundle treatment.Correlation analysis were respectively studied between Cys C and β2-MG concentration and APACHEⅡ scores within the two groups. Results APACHEⅡ score, Cys C and β2-MG concentration declined with the improvement of the disease in the survival group and it was opposite in the death group. It were obtained with Pearson correlation analysis that APACHEⅡ scores correlated positively to Cys C and β2-MG concentration. Conclusion Cys C and β2-MG concentration can be the index which evaluate therapeutic effect and prognosis of septic shock and can more accurately evaluate the severity and prognosis of septic shock combined with APACHEⅡ score.
