矮小症儿童GH激发试验及25-(OH)D水平的调查分析

中文摘要:目的 对矮小症儿童生长激素(growth hormone,GH)激发试验结果及体内维生素D(vitamin D,VitD)的营养状况进行调查分析,初步探讨矮小症儿童的病因。方法 对160例均符合身材矮小诊断标准的患儿进行GH精氨酸与可乐定激发试验,采用电化学发光免疫分析法进行血清GH、25羟维生素D[25-hydroxy vitamin D,25-(OH)D]检测。结果 生长激素完全缺乏8例,占5.0%;部分缺乏52例,占32.5%;生长激素不缺乏100例,占62.5%。可乐定激发试验的阳性率优于精氨酸(χ2=9.468,P=0.002),精氨酸激发的峰值多出现在30min(58.7%),而可乐定激发的峰值多出现在90min(48.1%)。157例疑似GHD儿童血清25-(OH)D水平为22.65±7.42ng/ml,普遍存在维生素D缺乏或不足。其中25-(OH)D缺乏者62例,占39.5%;25-(OH)D不足者69例,占43.9%;25-(OH)D充足者26例,占16.6%。结论 矮小症儿童要尽早明确病因,及时正确地给予诊断和治疗对提高患儿的身高有重要意义。

Analysis of Growth Hormone Provocation Test and 25-hydroxy Vitamin D Levels in Children with Short Stature
Abstract:Objective To investigate growth hormone provocation test and the nutritional status of vitamin D in children with short stature, so to study the etiology of children with short stature. Methods Totally 160 cases of short stature children who screened with diagnosis standard were underwent arginine and clonidine provocation test. Serum GH and 25-hydroxy vitamin D levels were determined with Electrochemiluminescence immunoassay. Results A total of 8 cases were found with GH deficiency, accounting for 5%. Fifty two cases were found with GH part deficiency, accounting for 32.5%.100 cases were found with no GH deficiency, accounting for 62.5%.The diagnostic value of clonidine provocation test was better than arginine test(χ2=9.468,P=0.002). The peak value of arginine provocation test mostly focused on time 30 min(58.7%),but clonidine provocation test mostly focused on time 90 min(48.1%).The level of 25-(OH)D in 157 cases children suspected GH deficiency was 22.65±7.42ng/ml. Vitamin D deficiency or insufficiency of these children was common. A total of 62 cases were found with deficiency of 25-(OH)D,accounting for 39.5%;and 69 cases were found with insufficiency of 25-(OH)D,accounting for 43.9%;and 26 cases were found with sufficiency of 25-(OH)D,accounting for 16.6%. Conclusion Early diagnosis and treatment in a timely and correct manner plays a significant role in proving short stature children height.
