| 中文摘要:目的 建立并评价抗CCP抗体、类风湿因子(RF)和C反应蛋白(CRP)对类风湿关节炎(RA)的最佳诊断界值。方法 选择2014年6月~2015年3月以RA组130例、非RA组130例和对照组100例作为研究对象,采用酶联免疫吸附法测定血清抗CCP抗体,采用免疫透射比浊法测定血清RF及CRP的浓度,应用受试者工作特征曲线(ROC曲线)确定抗CCP抗体、RF和CRP的最佳诊断界值并评价三者的诊断效能。结果 RA组和非RA组血清RF浓度分别为275.9±317.6和42.8±131.3IU/ml,前者显著高于后者(t=7.571,P<0.05);RA组和非RA组CRP浓度分别为39.2±47.3和29.7±42.2mg/L,前者显著高于后者(t=2.602,P=0.015)。抗CCP抗体、RF和CRP诊断RA的ROC曲线下面积(AUC)分别为0.868(0.820~0.916)、0.785(0.732~0.846)、0.587(0.512~0.661)。血清抗CCP抗体10RU/ml作为诊断界值时,诊断RA的敏感度、特异性和约登指数分别为0.669、0.892和0.562;RF以25IU/ml作为诊断界值时,诊断RA的敏感度、特异性、约登指数分别为0.715、0.777和0.492;CRP以30mg/L作为诊断界值时的敏感度、特异性和约登指数分别为0.469、0.623和0.092。结论 抗CCP抗体、RF、CRP的对RA的最佳诊断界值分别为10RU/ml、25IU/ml和30mg/L。抗CCP抗体诊断效能高于RF,而RF诊断效能高于CRP。 |

| Abstract:Objective To establish and assess the cut-off values of anti-cyclic citrullinated peptide antibody(anti-CCP), rheumatoid factor(RF) and C-reactive protein(CRP) in serum for rheumatoid arthritis(RA). Methods A total of 130 RA, 130 non-RA patients and 100 healthy people from June 2014 to March 2015 were enrolled. The concentration of anti-CCP was analyzed by ELISA, and the concentration of RF and CRP were analyzed by immunity transmission turbidity. The optimal diagnostic cut-off values of anti-CCP, RF and CRP for RA were determined by ROC, and the diagnostic performance of them were assessed. Results The concentration of RF in RA and non-RA group were 275.9±317.6IU/ml and 42.8±131.3IU/ml respectively and the former was significantly higher than the latter.(t=7.571, P<0.05).The concentration of CRP in the RA and non RA group were 39.2±47.3mg/L and 29.7±42.2mg/L respectively and the former was significantly higher than the latter(t=2.602, P=0.015). The area under ROC curve(AUC) of anti-CCP, RF and CRP were 0.868(0.820-0.916), 0.785(0.732-0.846), 0.587(0.512-0.661), respectively. For 30mg/L as cut-off value of CRP, sensitivity, specificity and Youden index were 0.469, 0.623 and 0.092, respectively. Conclusion The optimal diagnosis cut-off values of anti-CCP, RF and CRP were 10RU/ml, 25IU/ml, and 30mg/L, respectively. The diagnostic performance of anti-CCP was higher than that of RF, which was better than that of CRP. |
