胰高血糖素与2型糖尿病的相关性研究

中文摘要:目的 探讨2型糖尿病患者空腹及糖负荷后血浆胰高血糖素(Gg)、血糖、C肽的变化及三者之间的动态关系,并分析2型糖尿病患者血脂、体重指数(BMI)的变化及病程、年龄、BMI、空腹血糖(FBG)、糖化血红蛋白、血脂与胰高血糖素的相关性。方法 对照组和糖尿病组清晨空腹测量身高、体重后行糖耐量实验(OGTT),分别于空腹、糖负荷后30、60、120、180min测定血浆Gg、血糖、C肽的水平,并对Gg与C肽、糖化血红蛋白、血脂等指标进行相关性分析。结果 两组C肽和Gg的分泌量随着血糖的升降而升降。糖尿病患者空腹及餐后血浆Gg水平均高于对照组(P<0.05)。糖尿病组BMI、总胆固醇(CHOL)、甘油三酯(TG)、低密度脂蛋白(LDL)水平高于对照组,高密度脂蛋白(HDL)水平低于对照组(P<0.05)。糖尿病组,Gg与病程、年龄、BMI、糖化血红蛋白、FBG、CHOL、TG、LDL呈正相关,与HDL呈负相关。结论 Gg的绝对或相对分泌增多,是2型糖尿病患者高血糖症的因素之一。血糖、C肽、胰高血糖素相互促进分泌为三者之间的关系。在糖尿病患者中随着病程、年龄、BMI、血脂水平的增长,胰岛β细胞功能受损和Gg水平升高并存,对于保护胰岛β细胞功能和调控胰高血糖素双管齐下才能使血糖达到理想水平。

Correlation between Glucagon and Type 2 Diabetes Mellitus
Abstract:Objective To investigate the variation of plasma glucagon,blood sugar,C-peptide in patients with type 2 diabetes mellitus and dynamic relationship between them, and analyze the variation of lipid,BMI of type 2 diabetes patients. In addition, to analyze the correlation between glucagon and disease course,age,BMI,fasting blood glucose,glycosylated hemoglobin and lipid. Methods Subjects in healthy control group and diabetes group do an oral glucose tolerance test respectively after measuring height and weight on an empty stomach in the early morning, then those of both group were tested for plasma glucagon,blood glucose and C-peptide at fasting and 30, 60, 120 and 180 minutes after sugar load. The correlation analysis was made between glucagon and C-peptide、glycosylated hemoglobin and lipid. Results In both group, C-peptide and glucagon level was up and down with glucose.Diabetes group,fasting and postprandial plasma glucagon level was higher than those in control group (P<0.05). BMI,CHOL,TG and LDL level in diabetes group were higher than those in control group, while HDL level was on the contrary.In diabetes group, disease course,age,BMI,glycosylated hemoglobin,FBG,CHOL,TG and LDL had positive correlation with glucagon, however, HDL had negative correlation with glucagon. Conclusion The absolute or relative secretion of increased glucagon is one of the factors that lead to hyperglycemia in patients with type 2 diabetes. The relationship between glucose,C-peptide and glucagon is promoting secretion mutually. In diabetes group, with the growth of disease course,age and lipid level,the islet β-cell function damaged accompanied with high secretion of α-cell. In order to make blood glucose control at target easier, we need to protect β-cell function and pay attention to controlling α-cell function at the same time.
