性激素结合球蛋白与多囊卵巢综合征患者代谢综合征的相关性研究

中文摘要:目的 探讨多囊卵巢综合征(PCOS)患者中性激素结合球蛋白(SHBG)水平与代谢综合征(MetS)的相关性。方法 参照Rotterdam PCOS诊断标准纳入PCOS患者593例,测量PCOS患者临床特征、激素水平及糖脂代谢水平。结果 PCOS合并MetS女性与PCOS非MetS女性,SHBG水平具有显著差异;随着MetS诊断因素的增加,SHBG水平降低。根据SHBG水平的四分位数分组,随着SHBG水平的降低,PCOS患者MetS诊断因素、BMI、及糖脂代谢水平均有显著变化;PCOS患者血清SHBG水平与收缩压、舒张压、体重、体重指数、腰围、腰臀比、FPG、FINS、HOMA-IR、TG、LDL-C、apoB、apoB/A1及临床生化高雄指标mF-G评分、DHEAS、T、AND、FAI均存在负相关性,与HDL-C、apoA1存在正相关性。结论 低水平的SHBG与PCOS女性体内雄激素活性与代谢异常密切相关,SHBG水平可成为PCOS患者治疗过程中疗效评价的新指标。

Relationship between Sex Hormone Binding Globulin and Metabolic Abnormalities in Polycystic Ovary Syndrome Women
Abstract:Objective To investigate the relationship between sex hormone binding globulin(SHBG)and metabolic syndrome (MetS) in polycystic ovary syndrome (PCOS) women. Methods According to 2003 Rotterdam PCOS criteria, 593 PCOS subjects were recruited and were checked for clinical characteristics, hormone levels and glucolipid metabolism. Results The SHBG levels were significantly different between PCOS with or without MetS subjects. With increasing the number of MetS diagnostic components,the SHBG level became lowered progressively.According to SHBG levels of quartile groupings, with the reduction of SHBG levels, the MetS diagnostic factors, BMI, and glucolipid metabolism hadsignificant changes. The SHBG level was negatively associated with systolic pressure, diastolic blood pressure, body weight, body mass index, waist circumference, waist-hip ratio, FPG, FINS, HOMA-IR, TG, LDL-C, apoB, apoB/A1 and mF-G scores, DHEAS, T, AND, FAI levels, and positively with HDL-C and apoA1 concentrations. Conclusion The low level of SHBG was related to the androgen bioactivity and metabolic disturbance. The SHBG level may be the new indexes to assess the metabolic metabolic status.
