老年髋部骨折术后早期抗骨质疏松治疗的临床研究

中文摘要:目的 探讨老年骨质疏松性髋部骨折内固定术后早期应用阿仑膦酸钠、碳酸钙D3片及阿法骨化醇软胶囊抗骨质疏松治疗的临床疗效。方法 60例采用内固定手术治疗的老年骨质疏松性股骨转子间骨折患者随机分为治疗组和对照组各30例,治疗组术后2周开始应用阿仑膦酸钠、碳酸钙D3片及阿法骨化醇软胶囊抗骨质疏松治疗,对照组术后2个月开始应用阿仑膦酸钠、碳酸钙D3片及阿法骨化醇软胶囊抗骨质疏松治疗。所有患者均定期复查随访1年,评价疗效。结果 60例患者均获得随访1年,术后6个月时,治疗组腰椎的骨密度为0.721±0.132g/cm2,对照组腰椎的骨密度为0.635±0.127g/cm2,治疗组腰椎的骨密度高于对照组,差异有统计学意义(P<0.05)。术后1年时,治疗组腰椎的骨密度为0.741±0.139g/cm2,对照组腰椎的骨密度为0.668±0.131g/cm2,治疗组腰椎的骨密度高于对照组,差异有统计学意义(P<0.05)。治疗组的骨折愈合时间为11.533±1.548周,对照组的骨折愈合时间为11.800±1.690周,两组患者骨折愈合时间比较,差异无统计学意义(P>0.05)。结论 老年骨质疏松性髋部骨折术后早期服用阿仑膦酸钠、碳酸钙D3片及阿法骨化醇软胶囊抗骨质疏松治疗能提高骨密度,不影响骨折愈合。

Clinical Study of Postoperative Adjuvant Treatment of Anti-osteoporosis at the Early Stage in Elderly Patients with Hip Fracture
Abstract:Objective To analyze the clinical efficacy of postoperative adjuvant treatment of anti-osteoporosis with alendronate, calcium carbonate D3 and alfacalcidol at the early stage in elderly patients with hip fracture. Methods Sixty senile osteoporosis patients with intertrochanteric fracture after internal fixation operation were randomly divided into treatment group and control group, 30 cases in each group. Patients in the treatment group received the adjuvant treatment of anti-osteoporosis with alendronate, calcium carbonate D3 and alfacalcidol at 2 weeks after operation. Patients in the control group received the adjuvant treatment of anti-osteoporosis with alendronate, calcium carbonate D3 and alfacalcidol at 2 months after operation. All patients were followed-up regularly for 12 months to evaluate the clinical efficacy. Results All patients were followed up for 1 year. The bone mineral density (BMD) of lumbar were 0.721±0.132g/cm2 in treatment group and 0.635±0.127g/cm2 in control group at 6 months after operation. The BMD of lumbar were 0.741±0.139g/cm2 in treatment group and 0.668±0.131g/cm2 in control group at 12 months after operation. The BMD of the treatment group was higher than the control group, there was significant difference between the 2 groups (P<0.05). The healing time of fracture was 11.533±1.548 weeks in treatment group and 11.800±1.690 weeks in control group, there was no significant difference between the 2 groups (P>0.05). Conclusion The postoperative adjuvant treatment of anti-osteoporosis with alendronate, calcium carbonate D3 and alfacalcidol at the early stage in elderly patients with hip fracture can increase bone density, not influence healing time of fracture.
