脆性骨折及骨关节炎松质骨中经典Wnt通路相关基因表达差异的研究

中文摘要:目的 比较经典Wnt通路相关基因在股骨颈脆性骨折和髋关节骨关节炎病例中的表达差异,探讨相关基因对骨强度的影响。方法 入选病例分为脆性骨折(fragility fracture,FF)组及骨关节炎(osteoarthritis,OA)组,其中,FF组共14例,女性占71.4%,患者平均年龄74.64±6.98岁;OA组共13例,其中女性占76.9%,患者平均年龄66.9±6.98岁;所有标本均来自单侧髋关节置换术患者自愿捐献的新鲜股骨头;咬取股骨头与股骨颈交界中点并远离骨折线处的松质骨,提取总RNA并反转录成cDNA,使用实时荧光定量(quantitative real-time PCR,qRT-PCR)分别检测SOST、DKK-1、Ctnnb1、LRP5、FZD1等基因mRNA的表达水平。结果 脆性骨折组与骨关节炎组比较,SOST基因mRNA水平较高,而Ctnnb1较低,P均<0.05;DKK-1、LRP5、FZD1等基因的mRNA水平差异无统计学意义(P>0.05)。结论 骨强度的降低可能与SOST基因表达增强以及Ctnnb1基因表达的减弱有关。

Differential Expression of Classical Wnt Pathway Related-genes in Trabecular Bone of Fragility Fracture and Osteoarthritis
Abstract:Objective To compare the expression of classical Wnt pathway-related genes in femoral neck fragile fractures and hip osteoarthritis cases, and to explore the effects of related genes on bone strength. Methods Cases were divided into fragile fracture group (FF Group) and osteoarthritis group (OA Group). There were 14 cases in the FF group, 71.4% were female, and the average age was 74.64±6.98 years. 13 cases were in the OA group, 76.9% were female, and the mean age was 66.9±6.98 years. All femoral head specimens were obtained from patients who underwent total hip replacement surgery. The trabecular bone were taken from the midpoint between femoral head and femoral neck, and make sure far away from the fracture line. Then, the expression of SOST,DKK-1,Ctnnb1,LRP5,FZD1 were detected by quantitative real-time PCR. Results The expression of SOST in the fragile fracture group was significantly higher than that in the osteoarthritis group, while the expression of Ctnnb1 was lower (P <0.05). The expression of DKK-1, LRP5 and FZD1 had no significant differences. Conclusion The decrease of bone strength may be related to the enhancement of SOST expression and the decreased expression of Ctnnb.
