经淋巴结行MR淋巴管成像的初步实验研究

中文摘要:目的 初步探讨经淋巴结MR淋巴管成像的可行性。方法 新西兰大白兔8只,每只兔充分麻醉后直视下找到两侧腘窝最大的淋巴结分别注射0.2~0.3ml钆双胺,扫描冠状位三维扰相梯度回波序列(fast low angle shot,fl3d-cor),此后每隔5min重复一次扫描;分别评价腰淋巴干及胸导管的显示情况。结果 8例腰淋巴干全程均明显强化,均表现为下端与结节状的淋巴结相连,其中5例显示与其上端膨大的乳糜池相连;管径不均匀,约1~2mm,而且信号强度改变较明显。7例可见部分胸导管强化,其中4例显示范围>3个椎体高度,另3例不足3个椎体高度。强化的淋巴结及腰淋巴干、乳糜池等结构强化非常明显,通过后重建图像可明确显示它们互相连通。结论 两侧腘窝淋巴结注射钆双胺MRL能较好地显示腰干淋巴管,而胸导管只能节段显示,还需进一步探索以提高胸导管显示情况。

Initial Experience with Intranodal MR Lymphangiography in Rabbits
Abstract:Objective To demonstrate the feasibility of intranodal contrast material-enhanced MR lymphangiography (MRL) with popliteal lymph node injection of Gadodiamide. Methods A T1 weighted three-dimendional (3D) fast low angle shot (fl3d)sequence was utilised before and after administer Gadodiamide to undertake MRL. Gadodiamide (0.2-0.3ml per rabbit) was administered in the left and right popliteal lymph node in 8 rabbits, and then undertake MRL in 1.5T MRI system every 5min. Results The lumbar lymphatic trunk can be detected in all 8 rabbits, and parts of the thoracic duct can be detected in 7 rabbits. The enhanced lymph nodes, lymphatic trunk and cisterna chyli can be detected easily, which connect with each other in multiple planar reconstruction (MPR) imaging. Conclusion The lumbar lymphatic trunk can be detected very well with popliteal lymph node injection of MRL, but we have to do more to detect the thoracic duct better.
