| 中文摘要:目的 探讨鸦胆子油乳胸腔注入联合全身化疗治疗晚期非小细胞肺癌（NSCLC）所致恶性胸腔积液（MPE）的疗效及安全性。方法 选取2012年2月~2016年10月笔者医院收治的晚期NSCLC合并MPE 90例，随机分为两组各45例。对照组：仅予患者全身化疗，治疗组在对照组基础上，排净胸腔积液后向胸腔内注入鸦胆子油乳30ml，2次/周。比较两组胸腔积液吸收情况、胸腔积液病理及毒性不良反应发生情况。结果 治疗组胸腔积液控制率高于对照组（86.67% vs 64.44%），治疗组胸腔注药后胸腔积液病理阳性率显著降低，差异有统计学意义（P<0.05）。毒性不良反应主要为骨髓抑制、胃肠道反应、发热和胸痛，两组比较差异无统计学意义（P>0.05）。结论 胸腔注入鸦胆子油乳联合全身化疗治疗MPE，可有效杀灭肿瘤细胞、促进胸膜粘连及缓解症状，且不增加毒性不良反应。值得临床推广应用。 |

| Combining Usage of Chemotherapy and Intrapleural Injection with Brucea Javanica Oil in Malignant Pleural Effusion |

| Abstract:Objective To study their efficacy and safety of combining usage of chemotherapy and intrapleural injection with brucea javanica oil in treatment of malignant pleural effusion (MPE). Methods 90 NSCLC patients with MPE in Nanjing Chest Hospital between February 2012 and October 2016 were randomly divided into control and treatment group. Subjects in control group were treated with chemotherapy, while those in treatment group were chemotherapy plus intrapleural injection with brucea javanica oil. Both clinical efficacy and adverse reactions were compared. Results By Combining usage of brucea javanica oil, the treatment group has higher effective rate (86.67% vs 64.44%), lower pathology rate than control groups (P<0.05). The main toxicity, matological toxicity, gastrointestinal tract side reaction fever and chest pain was tolerated. There was no significant difference between two groups(P>0.05). Conclusion Combining usage of brucea javanica oil and chemotherapy in treatment of MPE was safe and effective, which can remarkably inhibit the growth of tumor, promote pleural adhesions and relief symptom. |
