| 中文摘要:目的 探讨外周血辅助型T22（Th22）细胞在评估脓毒症患者病情严重程度及预后中的临床意义。方法 选取2013年1月~2017年1月到笔者医院就诊的脓毒症患者220例为研究对象，根据脓毒症患者病情严重程度将其分为低危组（84例）、中危组（74例）和高危组（62例）。按脓毒症患者临床结局将其分为存活组（195例）和死亡组（25例）。检测各组外周血Th22细胞、白介素-22（IL-22）及C-反应蛋白（CRP）水平，同时记录急性生理学与慢性健康状况（APACHEⅡ）评分情况；采用受试者工作特征曲线（ROC）评估外周血Th22细胞对脓毒症死亡的预测价值。结果 低危组、中危组及高危组间外周血Th22细胞、IL-22及APACHEⅡ评分水平比较，差异有统计学意义（P<0.05）。其中，高危组最高，中危组次之，低危组最低。死亡组外周血Th22、IL-22水平及APACHEⅡ评分明显高于存活组，差异有统计学意义（P<0.05）。相关性分析显示，外周血Th22细胞与IL-22、APACHEⅡ评分呈正相关（IL-22：r=0.70，P<0.01；APACHEⅡ评分：r=0.75，P<0.01）。外周血Th22细胞>3.3%为评估脓毒症预后不佳界限时，其敏感度和特异性分别为84.4%和86.1%。结论 外周血Th22细胞及其IL-22与脓毒症病情严重程度及预后密切相关，其作为一项有效的评估指标，具有重要的临床运用价值。 |

| Clinical Significance of Th22 Cells in Assessing the Severity and Prognosis of Patients with Sepsis |

| Abstract:Objective To investigate the clinical significance of peripheral blood T-helper (Th22) cells in assessing the severity and prognosis of patients with sepsis.Methods A total of 220 patients with sepsis in our hospital from January 2013 to January 2017 were enrolled in the study. According to the severity of sepsis, these patients were divided into low risk group (84 cases), middle risk group (74 cases) and high risk group (62 cases). According to the clinical outcome of sepsis, these patients were divided into survival group (195 cases) and death group (25 cases). The levels of Th22 cells, IL-22 and CRP in the peripheral blood were measured, and the acute physiology and chronic health status (APACHE Ⅱ) score were recorded, the predictive value of peripheral blood Th22 cells in deathof sepsis was assessed by the receiver operating characteristic curve (ROC).Results The levels of Th22 cells, IL-22 and APACHEⅡ in the low-risk group, the intermediate-risk group and the high-risk group were statistically significant (P<0.05). The high-risk group was highest, followed by the intermediate-risk group, the low-risk group was lowest among them. The levels of Th22, IL-22 and APACHE Ⅱ scores of the death group were significantly higher than those in the survival group (P<0.05). The correlation analysis showed that the Th22 celss in peripheral blood were positively correlated with IL-22 (r=0.70, P<0.01) and APACHEⅡ scores (r=0.75, P<0.01). When Th22 cells in peripheral blood>3.3% for the assessing the poor prognosis of sepsis boundaries, the sensitivity and specificity were 84.4% and 86.1%.Conclusion The Th22 cells in peripheral blood and IL-22 are closely related to the severity and prognosis of sepsis, serving as an assessing index and have important clinical value. |
