不同孕期妊娠女性甲状腺激素TSH、FT3、FT4、TT3、TT4水平的变化与临床分析

中文摘要:目的 探究不同孕期妊娠女性甲状腺激素TSH、FT3、FT4、TT3、TT4水平的变化。方法 收集2016年1月~2017年1月无甲状腺相关病史的106例妊娠女性的临床资料,对不同孕期妊娠女性的TSH、FT3、FT4、TT3、TT4水平给予化学发光微粒子免疫分析法检测,并将同期在笔者医院行常规体检的100例健康非孕女性性作为对照组,比较上述水平变化情况。结果 与正常对照组比较,早、中、晚期妊娠组女性的TSH、FT3、FT4、TT3等甲状腺激素水平明显偏低(P<0.05),3组组间比较差异有统计学意义(P<0.05),早期、中期、晚期妊娠组女性TSH、TT3、TT4水平呈现上升趋势,FT3、FT4水平则逐渐下降;妊娠组女性甲状腺疾病发生率达到9.73%,明显高于正常对照组的6.0%,差异有统计学意义(P<0.05);晚期妊娠组女性ATG、TPOAb阳性率分别为4.8%、3.6%,显著低于中期与早期妊娠组(P<0.05),差异有统计学意义。结论 不同孕期妊娠女性甲状腺激素水平呈现出明显的差异性,应加强对孕产妇血清甲状腺激素水平的检查,及时筛查甲状腺功能,确保优生优育。

Changes and Clinical Analysis of Thyroid Hormones TSH,FT3,FT4,TT3 and TT4 Levels in Pregnant Women
Abstract:Objective To investigate the changes of TSH, FT3, FT4, TT3 and TT4 levels in pregnant women with different gestational age. Methods The data of TSH, FT3, FT4, TT3and TT4 were analyzed by chemiluminescence microparticle immunoassay in the pregnant women with different thyroid-related history from January 2016 to January 2017. The levels of TSH, FT3, And the same period in our hospital routine physical examination of 100 healthy non-pregnant women as a control group, compare the above level changes. Results Compared with the normal control group, the levels of TSH, FT3, FT4, TT3 and other thyroid hormones in the early, middle and late pregnancy groups were significantly lower (P<0.05), and there was significant difference between the three groups (P<0.05). The levels of TT3, TT4 and TSH in the early, middle and late stage pregnant women showed an increasing trend, and the FT3 and FT4 levels decreased gradually. The prevalence of thyroid disease in pregnant women was 9.73%, which was significantly higher than that in the normal control group (P<0.05). The positive rates of ATG and TPOAb in the late pregnancy group were 4.8% and 3.6%, respectively, which were significantly lower than those in the middle and early pregnancy groups (P <0.05), and the difference was statistically significant. Conclusion Thyroid hormone levels of pregnant women in different gestures show obvious differences, should strengthen the serum thyroid hormone levels in pregnant women check the timely screening of thyroid function to ensure prenatal and postnatal care.
