超声斑点追踪技术对冠心病支架置入术后左心室收缩功能的评估价值

中文摘要:目的 探讨超声斑点追踪技术(speckle tracking imaging,STI)在冠心病支架置入术(percutaneous coronary intervention,PCI)后左心室局部与整体收缩功能评价中的应用价值。方法 选取2017年3月~2017年8月在浙江省人民医院心内科经PCI治疗的冠心病患者60例为病例组,笔者医院行健康体检人员60例为对照组。病例组分别于手术前3天、术后1、2、5个月时行超声心动图与超声斑点追踪技术检查,比较病例组术前术后与对照组常规超声心动图参数、斑点追踪技术二维应变参数。结果 病例组术前常规超声心动图参数(A、E/A、LVEF)与斑点追踪技术二维应变参数(GBLS、GBRS和GBCS)均低于对照组(P<0.05);手术后1、2、5个月LVEF比术前增高,术后2、5个月E/A值比术前增高(P<0.05);术后1、2、5个月的GBLS、GBRS和GBCS比术前均有提高(P<0.05),各参数术后均呈递增趋势升高。二维整体应变参数(GBLS、BGRS和GBCS)与LVEF (Simpson's双平面法)均存在相关性(P<0.05)。结论 STI可以定量、客观地评价冠心病患者PCI术前术后心肌局部及整体收缩功能,为临床决策治疗和评估预后提供量化指标。

Evaluation of Speckle Tracking Imagingin Left Ventricular Systolic Function in Patients with Coronary Heart Disease before and after PCI
Abstract:Objective To explore value of speckle tracking imagingin left ventricular systolic function in patients with coronary heart disease before and after PCI. Methods Sixty patients with coronary heart disease by PCI and 60 healthy people admitted to our hospital from March 2017 to August2017 were enrolled in this study, respectively.The patients underwent echocardiography and STI 3 days before PCI and 1,2,5 months after PCI. Comparison of parameters of routine echocardiography and STI between patients before and after PCI and healthy people was performed. Their correlation were compared. Results Before PCI, the parameters of routine echocardiography(A,E/A,LVEF) and STI (GBLS,GBRS,GBCS) were significantly lower in patient group than in control group(P<0.05), and significantly higher in patient group 1,2 and 5 months after PCI than in control group(P<0.05). The GBLS,GBRS,and GBCS were significantly correlated with LVEF (Simpson's biplane method) (P<0.05). Conclusion STI can quantitatively evaluate the regional and globalsystolic function of left ventricle in patients with coronary artery disease before and after PCI, and provide quantitative indicators for clinical decision-making and prognosis.
