RA患者血清TNF-α和外周血单个核细胞TIPE2表达的意义

中文摘要:目的 探讨类风湿关节炎(rheumatoid arthritis,RA)患者血清中肿瘤坏死因子α(tumor necrosis factor,TNF-α)和外周血单个核细胞中肿瘤坏死因子α诱导蛋白家族8样蛋白2(tumor necrosis factor-α induced protein 8 like 2,TIPE2)的表达在RA的诊断及疗效评价中的价值。方法 选取开封市第二人民医院风湿科近两年收治的RA患者86例,包括活动期患者42例,缓解期患者44例,门诊健康对照40例,用酶联免疫吸附法检测3组的血清中TNF-α的浓度,无菌抽取各组外周血,提取外周血单个核细胞,用荧光定量PCR及Western blot法检测TIPE2在mRNA及蛋白水平的表达。结果 RA活动组和缓解组患者血清中的TNF-α浓度高于健康对照组,RA活动组血清中TNF-α明显高于缓解组,差异有统计学意义(P<0.05)。TNF-α和RF联合检测的敏感度和特异性分别为75.5%和90%,特异性与单独检测RF相比,差异有统计学意义(P<0.05)。RA活动期患者血清中TNF-α的浓度及TIPE2在mRNA表达水平和DAS28评分呈正相关(P<0.01),RA患者血清中TNF-α的浓度和TIPE2在mRNA表达水平呈正相关(P<0.01)。RA患者外周血TIPE2在mRNA及蛋白水平均高于对照组,而且活动组高于缓解组(P<0.01)。结论 联合检测RA患者血清中的TNF-α对患者的诊断和病情的监测有重要临床价值,TIPE2参与了RA的发病过程,可以和TNF-α一起作为判断疾病严重程度的指标。

Clinical Values of TNF-α in Sera and TIPE2 in Peripheral Blood Lymphocytes for Rheumatoid Arthritis
Abstract:Objective To investigate the clinical values of TNF-α in sera and TIPE2 in peripheral blood mononuclear cells for the diagnosis and therapeutic evaluation of rheumatoid arthritis. Methods Serum samples and peripheral blood mononuclear cells were obtained from 86 RA patients and 40 normal controls in the NO.2 Hospital of Kaifeng. The patients were divided into two groups including 42 active RA patients and 44 inactive RA patients. The concentration of serum TNF-α were measured by ELISA test. The TIPE2 mRNA and protein expression was measured by quantitative RT-PCR and Western blot. Results The concentration of serumTNF-α in RA patients were higher than in healthy controls(P<0.01). The concentration of serumTNF-α in active RA patients were higher than in inactive RA patients(P<0.01). The specificity and sensitivity of combined detection RF and TNF-α were 90% and 75.5% which were higher than single detection. The concentration of serum TNF-α and mRNA level of TIPE2 was positively correlated to DAS28. The concentration of serum TNF-α was positively correlated to mRNA level of TIPE2. The mRNA and protein level of TIPE2 in RA patients were higher than in healthy controls(P<0.01). The mRNA and protein level of TIPE2 in active RA patients were higher than in inactive RA patients(P<0.01). Conclusion Combined detection of RF and TNF-α had high diagnostic value and contributed to estimate the clinical observation. TIPE2 plays a role in the RA pathogenesis and TIPE2 may be a surrogate for severity of rheumatoid arthritis.
