胸段食管鳞癌调强放疗过程中肿瘤退缩情况观察

中文摘要:目的 观察胸段食管鳞癌在调强放疗过程中, 食管原发肿瘤在三维方向上的退缩和位移情况, 探讨食管癌放疗过程中是否有必要因肿瘤靶区退缩和位移而重新定位修改放疗靶区。方法 随机选择首程接受调强放疗联合同步化疗治疗局部进展期胸段食管鳞癌患者33例, 放疗总剂量63Gy/30次, 6周完成。在治疗前、21Gy/10次、42Gy/20次和63Gy/30次分别进行食管CT检查, 测量肿瘤放疗不同时段在左右界、前后界、上下界3个方向上的退缩和位移。结果 肿瘤退缩从大到小顺序在放疗不同时段依次为放疗20次、前10次和后10次, 在三维方向依次为上下长度、前后长度和左右长度;肿瘤中心位移从大到小顺序在放疗不同时段依次为放疗20次、30次和前10次, 在三维方向依次为左右方向、前后方向和上下方向。肿瘤中心位移与病变部位无明显相关性(P > 0.05)。结论 在不同放疗次数时肿瘤在三维方向上均有缩小和左右及前后方向的肿瘤中心位移, 其中放疗20次时最为明显。

Tumor Regression of Thoracic Esophageal Squamous Cell Carcinoma during Intensity Modulated Radiotherapy
Abstract:Objective To observe the primary tumor regression and the displacement of thoracic esophageal squamous cell carcinoma (TE-SCC) in the three-dimensional direction during intensity modulated radiotherapy (IMRT), and to probe whether it is necessary of the reposition and the modification of tumor target because of tumor regression and the displacement during IMRT. Methods 33 local advanced TE-SCC patients randomly selected underwent the first course of IMRT concurrent with chemotherapy. The total dose was 63Gy in 30 fractions in 6 weeks. CT scans of esophagus were done before therapy, in 21Gy/10Fx, in 42Gy/20Fx and in 63Gy/30Fx respectively. The tumor regression and the displacement were detected the three-dimensional direction. Results Ranked from large to small order, tumor regressions were during in 42Gy/20 Fx, in 21Gy/10Fx and in 63Gy/30Fx respectively; and in up and down direction, in front and back direction and in right and left direction respectively. There was no distinct correlation between the displacement and the tumor location (P>0.05). Conclusion During different fractions of IMRT, the tumor regression occurred in three-dimensional direction, and there were the displacements in right and left direction and in front and back direction. It was especially evident in 42Gy/20Fx.
