中文摘要:目的 探讨血浆NT-proBNP检测在AECOPD合并Ⅱ型呼吸衰竭患者机械通气治疗中的临床应用价值。 方法 选择中心ICU进行有创机械通气48h以上、临床诊断为慢性阻塞性肺疾病急性加重(AECOPD)合并Ⅱ型呼吸衰竭患者45例为研究对象,监测患者的血浆NT-proBNP值,动脉血气分析及APACHEⅡ评分,分析之间的相关性。比较患者机械通气治疗前后的血浆NT-proBNP值,动脉血气分析及APACHEⅡ评分的变化,观察其随病情变化的规律。评价血浆NT-proBNP在AECOPD合并Ⅱ型呼吸衰竭患者诊断,病情监测及预后评估中的临床意义。 结果 相关性分析表明,患者血浆NT-proBNP值分别与PaCO2和APACHEⅡ评分呈正相关,与PaO2呈负相关,差异均具有统计学意义。患者治疗后血浆NT-proBNP值、APACHEⅡ评分和血气分析指标等均较治疗前有明显改善,差异有统计学意义(P<0.05)。 结论 血浆NT-proBNP水平检测可作为预测AECOPD肺功能和预测AECOPD合并呼吸衰竭患者机械通气效果的指标,在AECOPD呼吸衰竭的诊断、病情严重程度以及发展变化的预测中具有较好的临床价值。

Clinical Value of Plasma NT-proBNP Detection in Patients Suffer from AECOPD Merged with Type Ⅱ Respiratory Failure
Abstract:Objective To study the clinical value of plasma NT-proBNP detection in patients who suffer from AECOPD merged with type Ⅱ respiratory failure and treated with mechanical ventilation. Methods Totally 45 patients diagnosed as AECOPD merged with type Ⅱ respiratory failure were selected as the study subjects. All selected patients have experienced over 48 hour's invasive mechanical ventilation in ICU. The NT-proBNP value, arterial blood gas analysis and APACHEⅡ score of these patients were detected, respectively. After that, the correlation between the above three indexes was analysed. The changes of plasma NT-proBNP values, arterial blood gas analysis and APACHEⅡ score before and after treatment were measured to evaluate the rule of disease variety in these patients. In addition, the clinical value of plasma NT-proBNP detection in patients with AECOPD and type Ⅱ respiratory failure was assessed. Results Correlation analysis showed that NT-proBNP values were positively correlated with PaCO2 and APACHEⅡ score, while negatively correlated with PaO2, and the difference was statistically significant (P<0.05). Plasma NT-proBNP values, blood gas analysis, APACHEⅡscores detected in these patients were improved significantly after treatment (P<0.05). Conclusion Detection of plasma NT-proBNP values can be used to predict the lung function and the effect of mechanical ventilation in patients with AECOPD. The detection of NT-proBNP values was valuable in the diagnosis and prediction of AECOPD merged with respiratory failure.
