T-SPOT TB联合IL-10、IL-27对结核性胸膜炎的诊断价值

中文摘要:目的 评价T-SPOT TB联合IL-10、IL-27检测对结核性胸膜炎的诊断价值。方法 纳入结核性胸膜炎患者42例和恶性胸腔积液患者44例,抽取患者胸腔积液进行T-SPOT TB检测,并用ELISA法检测IL-10和IL-27的表达水平。结果 结核性胸膜炎组患者中T-SPOT TB阳性39例,阴性3例,阳性率92.86%;恶性胸腔积液组患者中T-SPOT TB阳性2例,阴性42例,阳性率4.55%。与后者相比,结核性胸膜炎组患者T-SPOT TB检测阳性率较高,且二者差异有统计学意义(P<0.01)。结核性胸膜炎患者胸腔积液IL-10(142.86±33.12)和IL-27(418.20±63.4)表达水平,显著高于恶性胸腔积液组IL-10(96.18±13.41)和IL-27(127.33±32.86),差异具有统计学意义(P<0.05)。结论 T-SPOT TB联合测定IL-10、IL-27表达水平在结核性胸膜炎的诊断和鉴别诊断中具有重要价值。

Test of T-SPOT TB Combined with Cytokine IL-10, IL-27 in Diagnosis of Tuberculous Pleurisy
Abstract:Objective To evaluate the value of T-SPOT TB combined with test of IL-10 and IL-27 in diagnosis of tuberculous pleurisy. Methods Forty two patients with tuberculous pleurisy and 44 patients with malignant pleural effusion were included joining the T-SPOT TB test, and with enzyme-linked immuno sorbent assay (ELISA) method the expression level of IL-10 and IL-27 in the patients' pleural effusion was detected. Results In patients with tuberculous pleurisy group, T-SPOT TB test results were positive cases(n=39), negative cases(n=3),and the positive rate was 92.86%.In patients with malignant pleural effusion group, T-SPOT TB test results were positive cases(n=2), negative cases(n=42), and the positive rate was 4.55%. Tuberculous pleurisy patients' T-SPOT TB testing positive rate was obviously higher than the latter group, and there was a statistical significant differernce in two groups(P<0.01). In patients with tuberculous pleurisy group, the concentration of IL-10 and IL-27 in pleural effusions were 142.86±33.12 and 418.20±63.4. In patients with malignant pleural effusion group, the concentration of IL-10 and IL-27 in pleural effusions were 96.18±13.41 and 127.33±32.86; and there was statistically significant difference between the two groups(P<0.05, P<0.01). Conclusion T-SPOT TB combined with test of IL-10 and IL-27 had significant application value in the diagnosis and differential diagnosis of tuberculous pleurisy.
