| 中文摘要:目的 观察可活动型克氏针外固定牵引架治疗近节指间关节骨折脱位的疗效。方法 自2011年2月~2013年9月,应用手部自制克氏针-橡皮筋可活动型外固定牵引架治疗近节指间关节骨折脱位患者12例,均为新骨折,其中闭合性骨折11例,开放性骨折1例,单纯性骨折11例,粉碎性骨折1例。结果 12例患者均获得随访,随访时间6~26个月,平均随访时间11.5个月。12例患者均达到完全骨性愈合,愈合时间为8~16周,平均愈合时间12周,无骨不愈、针道感染并发症。手部功能恢复按TAM系统评定标准,优7例,良3例,可2例,优良率为83.3%。结论 可活动型外固定牵引架能够对近节指间关节骨折脱位起到稳定固定,利于术后早期功能锻炼及恢复,是一种简单、有效的治疗方法。 |

| Applicaion of Pins External Distraction Fixator in Treating Fracture-dislocation of Proximal Interphalangeal Joint |

| Abstract:Objective To observe the results of fracture of proximal interphalangeal joint,especially the intra-articular fracture-dislocation,treated with a dynamic external distraction fixator. Methods From February 2011 to Sebtember 2013,12 cases of fracture-dislocation of proximal interphalangeal joint treated with a dynamic external distraction fixator which made by pins and rubber band. All cases were fresh fracture,included 11 open fractures and 1 closed fracture,simple fracture in 11 cases and comminuted fracture in 1 case. Results Twelve cases were followed-up from 6 to 26 monthes,with a mean period of 11.5 monthes. All 12 cases achieved compete healing,and the average healing period was 12 weeks(range,8-16 weeks) and there was no complication on nonunion or pin-track infection. Hand function was assessed according to the total active movement(TAM),excellent in 7,good in 3,fair in 2,and the excellent and good rate was 83.3%. Conclusion The dynamic external distraction fixator not only has a rigid fixation for fracture-dislocation of proximal interphalangeal joint,but be beneficial to postoperative exercise and recovery. It is a kind of simple,effective treatment device. |
