中文摘要:目的 探讨急性心肌梗死(AMI)患者血清尿酸(UA)水平与心功能评价指标的相关性。方法 入选2015年1~12月入院的AMI患者157例,根据血清UA水平分为高尿酸血症组(n=41)和尿酸正常组(n=116)。比较两组患者一般资料、血液检测指标、超声心动图各指标及Grace危险评分,并分析UA与各指标的相关性。结果 高尿酸血症组体重指数(BMI)、合并高血压病比例、甘油三酯(TG)、N末端脑钠肽前体(NT-proBNP)、Grace评分以及左心室收缩末期容积(LVESV)较尿酸正常组高(P<0.05或P<0.01),而左心室射血分数(LVEF)及左心室短轴缩短分数(FS)显著降低(P<0.05)。单因素相关分析结果显示,高尿酸组尿酸水平与年龄、LDL-C、NT-ProBNP、Grace评分、LVESV和左心室舒张末期容积(LVEDV)呈正相关(P<0.05),与LVEF呈负相关(P<0.05)。结论 AMI合并高尿酸血症患者左心室收缩功能受损更明显,院内死亡风险更高,提示预后更差。 |

Study of the Relationship Between Serum Uric Acid Level and Cardiac Function in Acute Myocardial Infarction Patients |

Abstract:Objective To investigate the relationship between serum uric acid(UA) concentration and cardiac function in patients with acute myocardial infarction(AMI). Methods A total of 157 AMI patients in Beijing friendship hospital from January 2015 to December 2015 were enrolled and were divided into high UA group(n=41) and normal UA group(n=116).Clinical features,blood tests, echocardiogram and Grace risk scores were compared between the two groups. Results The body mass index, proportion of hypertension, triglyceride, N-terminal B-type natriuretic peptide (NT-proBNP), GRACE risk score and left ventricular end systolic volume(LVESV) were significantly higher in high UA group than normal UA group (P<0.05 or P<0.05) and left ventricular ejection fraction (LVEF) and left ventricular fractional shortening(FS) were significantly lower(P<0.05). Morever, serum UA concentration in the high UA group was positively related with age, low density lipoprotein-cholesterol(LDL-C), NT-proBNP,GRACE risk score, left ventricular end diastolic volume(LEVDV) and LVESV (P<0.05),and was negatively related with LVEF and FS(P<0.05). Conclusion AMI patients with high level of UA have more serious impairment of left ventricular systolic function, higher risk of in-hospital death and poor prognosis. |
