| 中文摘要:目的 分析探讨重症胰腺炎患者血乳酸水平与APACHEⅡ评分的相关性及指导预后的意义。方法 回顾性分析45例入住笔者医院重症医学科（intensive care unit，ICU）中重症胰腺炎患者的临床资料，根据其预后将患者分为存活组和死亡组，比较血乳酸水平与APACHEⅡ分值之间的关系，分析上述指标与患者预后之间的关系。结果 死亡组患者的血乳酸水平、APACHEⅡ评分明显高于存活组（P<0.05），APACHEⅡ评分>30分组血乳酸水平明显高于21~30分组（P<0.05），APACHEⅡ评分21~30分组血乳酸水平明显高于≤10及其11~20分组（P<0.05）；血乳酸水平和APACHEⅡ分值的相关性呈显著正相关（r=0.995，P<0.01）。结论 血乳酸水平、APACHEⅡ评分均与重症胰腺炎患者病情严重程度相关，在该类患者预后判断中具有一定的临床指导意义。 |

| Value of APACHEⅡ Scoring System and Blood Lactate in Assessing Prognosis in Patients with Severe Acute Pancreatitis |

| Abstract:Objective To analyze the correlation of blood lactate level and APACHEⅡ score, and their significance in guiding the prognosis of severe acute pancreatitis patients.Methods The clinical data of 45 patients with severe pancreatitis, admitted in intensive care unit (ICU), were retrospectively analyzed. The patients were divided into two groups:the survival group and the death group according to prognosis. We made comparisons between different blood lactate levels and different APACHEⅡ scores, and its correlation with the prognosis of patients was analyzed.Results Blood lactate level and APACHEⅡ scores in the death group were significantly higher than those in the survival group (P<0.05). The blood lactate levels in patients with APACHEⅡ score> 30 group were significantly higher than 21-30 group (P<0.05). These indexes in patients with APACHEⅡ score 21-30 group were significantly higher than the 11-20 group and ≤ 10 group (P<0.05). The blood lactate levels were positive correlated with APACHEⅡ scores (r=0.995, P <0.01).Conclusion Blood lactate levels and APACHEⅡ scores of patients with severe pancreatitis severity were associated with the disease severity, and has certain clinical value in predicting the prognosis of such critical illness. |
