| 中文摘要:目的 研究糖皮质激素治疗急性呼吸窘迫综合征（acute respiratory distress syndrome，ARDS）的近期预后。方法 回顾性分析中国医学科学院肿瘤医院重症医学科（intensive care unit，ICU）2013年1月~2015年12月间收治的全部ARDS患者的资料。根据患者是否使用糖皮质激素分为激素组和非激素组。结果 52例患者中男性40例，女性12例。平均年龄63.25±11.09岁。全组60天病死率21.1%（11/52）。激素组和非激素组的一般资料如年龄、性别、合并症评分、ARDS的分级、ARDS的危险因素、患者的病情危重程度比较差异无统计学意义（P>0.05）。两组患者的ICU病死率（12.5% vs 11.4%，P=0.926）和60天病死率（25.0% vs 20.5%，P=0.772）比较差异无统计学意义。两组患者第3天的氧合指数均较第1天显著改善。结论 应用糖皮质激素不能很好地改善ARDS患者的60天病死率。 |

| Effect of Corticosteroids on the Short-term Outcome of Acute Respiratory Distress Syndrome |

| Abstract:Objective To investigate the effect of corticosteroids on the short-term outcome of acute respiratory distress syndrome (ARDS).Methods The data of ARDS patients who admitted to intensive care unit (ICU) of National Cancer Center/Cancer Hospital, Chinese Academy of Medical Sciences and Peking Union Medical College between January 2013 and December 2015 were retrospectively reviewed and analyzed. Patients were divided into two groups:steroid group and non-steroid group.Results There were 52 ARDS patients according to Berlin criteria, and there were 40 males and 12 females, with a median age of 63.25±11.09. The 60-day mortality was 21.1% (11/52). There were no significant differences in age, gender, Charlson score, classification of ARDS, risk factors of ARDS, and severity of illness between steroid group and non-steroid groups. And there were no significant differences in ICU mortality (12.5% vs 11.4%, P=0.926) and 60-day mortality (25.0% vs 20.5%, P=0.772) between two groups. Oxygenation index (PaO2/FiO2) was significantly improved in 3rd day compared with 1st day in both groups (172±56 vs 106±65 in steroid group, and 199±55 vs 142±59 in non-steroid group).Conclusion Corticosteroids use does not improve 60-day mortality of ARDS patients. |
