| 中文摘要:目的 探讨急性心肌梗死患者CYP2C19、ABCB1基因多态性与血小板抵抗的相关性。方法 自2016年1月~2017年6月，连续性收集笔者医院收治的急性心肌梗死患者163例，测定患者是否为阿司匹林抵抗或氯吡格雷抵抗，并分析其与患者CYP2C19、ABCB1基因多态性的相关性。结果 阿司匹林抵抗和阿司匹林半抵抗的患者CYP2C19基因rs4244285和rs4986893呈明显多态性分布，突变型基因频率均显著高于阿司匹林敏感组（P<0.05）。氯吡格雷抵抗的患者CYP2C19基因rs4244285和ABCB1基因rs2235048呈明显多态性分布，突变型基因频率均显著高于氯吡格雷敏感的患者[（37.50% vs 13.64%，P=0.000）和（18.75% vs 9.60%，P=0.017）]。结论 阿司匹林抵抗与CYP2C19基因rs4244285和rs4986893突变有关，氯吡格雷抵抗与CYP2C19基因rs4244285和ABCB1基因rs2235048突变有关。 |

| Correlation Analysis of CYP2C19, ABCB1 Gene Polymorphism and Platelet Resistance in Patients with Acute Myocardial Infarction |

| Abstract:Objective To investigate the correlation between CYP2C19, ABCB1 gene polymorphism and platelet resistance in patients with acute myocardial infarction. Methods From January 2016 to June 2017, 163 consecutive patients with acute myocardial infarction in our hospital were collected. The patients with resistance to aspirin or clopidogrel were detected so as to study their association with CYP2C19, ABCB1 gene polymorphism. Results CYP2C19 and rs4986893 were significantly distributed in patients with aspirin resistance and aspirin half-resistance, and their frequencies of mutant genes were both significantly higher than that of rs4244285 sensitive group (P<0.05). CYP2C19 gene rs4244285 and ABCB1 gene rs2235048 in patients with clopidogrel resistance had obvious polymorphism distribution, and their frequencies of mutant gene were both significantly higher than that of clopidogrel sensitive patients[(37.50% vs 13.64%, P=0.000) and (18.75% vs 9.60%, P=0.017)]. Conclusion Aspirin resistance is associated with mutations in the CYP2C19 gene rs4244285 and rs4986893, and clopidogrel resistance is associated with the mutation in the CYP2C19 gene rs4244285 and the ABCB1 genes rs2235048 |
