| 中文摘要:目的 探索自身免疫性疾病与不良妊娠结局的相关性并构建预测模型。方法 回顾性分析2022年9月～2024年9月昆明医科大学第二附属医院产科250例妊娠合并自身免疫性疾病患者,根据患者妊娠结局分为妊娠结局良好组(n=117)和妊娠结局不良组(n=133),分析妊娠合并自身免疫性疾病患者发生不良妊娠结局的相关因素,并构建列线图预测模型。结果妊娠合并自身免疫性疾病患者的体重指数高、抗干燥综合征抗原B抗体阳性、子痫前期、白细胞计数高、D-二聚体水平高是发生不良妊娠结局的独立危险因素。基于分析结果构建的妊娠合并自身免疫性疾病患者发生不良妊娠结局风险的列线图预测模型具有一定区分度。结论 妊娠合并自身免疫性疾病患者发生不良妊娠结局的概率较高,与患者的体重指数、抗干燥综合征抗原B抗体阳性、子痫前期、白细胞水平、D-二聚体水平相关,基于此分析结果构建的列线图预测模型,可以在一定程度上评估妊娠合并自身免疫性疾病患者发生不良妊娠结局的概率。 |

| Establishment of Predictive Models for Autoimmune Diseases and Adverse Pregnancy Outcomes |

| Abstract:Objective To explore the correlation between autoimmune diseases and adverse pregnancy outcomes and establish a predictive model. Methods A retrospective analysis was conducted on 250 cases of pregnancy complicated with autoimmune diseases in the Obstetrics Department of the Second Affiliated Hospital of Kunming Medical University from September 2022 to September 2024. The patients were grouped according to their pregnancy outcomes, including 117 cases in the good pregnancy outcome group and 133 cases in the poor pregnancy outcome group. The related factors of adverse pregnancy outcomes in patients with pregnancy complicated with autoimmune diseases were analyzed. And construct the nomogram prediction model. Results High body mass index, positive anti-Sjogren′s syndrome antigen B antibody, preeclampsia, high leukocyte and D-dimer were independent risk factors for adverse pregnancy outcomes. Based on the analysis results, the nomogram prediction model for the risk of adverse pregnancy outcomes in pregnant patients with autoimmune diseases has a certain degree of differentiation. Conclusion There is a high probability of adverse pregnancy outcomes in pregnant patients with autoimmune diseases, among which BMI, positive anti-SSB, preeclampsia, white blood cell level and D-dimer level of patients are correlated. Based on the analysis results, the prediction model based on the histogram can evaluate the probability of adverse pregnancy outcomes to a certain extent. |
