| 中文摘要:目的研究锁骨下静脉走向、长度等参数，探索锁骨下静脉穿刺的最佳位置。方法40例经甲醛固定的尸体标本，取仰卧位，上肢自然平置于躯干两侧。解剖观察锁骨下静脉的行程，测量与该静脉有关的数据。结果①锁骨下静脉长度：男性左侧平均值长3.24cm，右侧平均值长2.87cm；女性左侧平均值长2.63cm，右侧平均值长2.31cm，左侧较右侧长（P<0.05）；②锁骨下静脉距锁骨下缘的垂直距离：男性左侧2.12cm、右侧1.95cm；女性左侧2.63cm、右侧1.92cm，左侧较右侧无显著性差异（P>0.05）；③头臂静脉与上腔静脉纵轴的夹角；男性左侧平均值为44.51°、右侧平均值为26.36°；女性左侧平均值为47.75°、右侧平均值为25.21°。不同性别之间差异无显著性（P>0.05），但左侧夹角较右侧大（P<0.001）。结论锁骨下静脉与锁骨下缘交叉，且位置表浅，前面仅覆以皮肤，皮下组织和锁骨下肌，只要掌握好要点，完全可以避免并发症的发生。建议右锁骨下静脉穿刺点应选在锁骨下缘中内1/3交界处。 |

| Clinical Anatomical Study on the Parameters of Subclavian Vein |

| Abstract:ObjectiveTo clarify the optimal position of venous puncture by measuring the parameters (trace, length) of subclavian vein. MethodsForty corpses, infused with formalin, were included in present study. The parameters of subclavian vein were determined in corpses which were supinely positioned with upper limbs naturally lying on the body. Results①The total length of ubclavian vein was about 3.24cm on the left and 2.87cm on the right in male and 2.63cm and 2.31cm in female respectively. They were significantly different between both sides (P<0.05) in either male or female. ②The vertical gap between subclavian vein and inferior margin of clavicle was not significantly different between left and right (left 2.12cm vs. right 1.95cm, left 2.63cm vs. right 1.95cm, P>0.05).3. The angle between brachiocephalic vein and superior vena cava was not significantly different between man and women (left: 44.51°in male vs. 47.75°in female; right: 26.36° in male vs. 25.2° in female, P>0.05). Notable, the left angle was significantly larger than that of right (P<0.001). ConclusionSubclavian vein crosses with clavicle and is covered with thin tissues (skin, subcutis and subclavius). It is suggested that the puncture site should locate at the crossover of inner and middle third clavicle. |
