| 中文摘要:目的 通过临床回顾性分析,寻找可能影响肺腺癌患者预后的因素。方法 回顾笔者医院2001年11月~2013年12月616例肺腺癌患者的临床资料,包括患者性别、年龄、PS评分、吸烟史、肿瘤分化程度、治疗方法、临床分期、合并疾病以及p53、Ki67、耐药基因表达,以电话联系的方式进行随访;采用Kaplan-Meier法计算生存率、COX模型进行多因素分析。结果 616例肺腺癌患者的1、2、3年生存率分别是37.5%,17.8%和8.4%。总的中位生存期为8.8个月,COX分析显示性别、临床分期、肿瘤分化程度、化疗与否、放疗与否、靶向治疗与否、表达Ki67高低及是否存在呼吸系统合并症是影响肺腺癌患者预后的独立因素。结论 临床分期、肿瘤分化程度、Ki67高表达、是否治疗及是否有呼吸系统合并症是影响肺腺癌预后的重要因素。 |

| Clinical Characteristics and Prognostic Factors of 616 Cases of Patients with Lung Adenocarcinoma |

| Abstract:Objective To identify the clinical characteristics and possible prognostic factors of patients with lung adenocarcinoma by retrospective analysis. Methods The data from 616 cases lung adenocarcinoma patients who were first diagnosed and treated in Changhai Hospital between November 2001 and December 2013 were retrospectively analyzed. The survival rates were calculated using the Kaplan-Meier method. The log-rank method was used for univariate analysis, and the Cox regression model was used for multivariate analysis. Results The one-year,two-year and three-year survival rates of the patients were 37.5%,17.8% and 8.4%. The median survival time was 8.8 months. In multivariate analysis, gender, clinical stage, tumor differentiation, chemotherapy, radiotherapy, therapy of EFGR-TKI, the expression of Ki67 were independent prognostic factors. Conclusion Gender,clinical stage,tumor differentiation, reception of treatment, the expression of Ki67 were independent prognostic factors. |
