H型高血压与颈动脉粥样硬化斑块特点的相关性分析

中文摘要:目的 分析H型高血压与颈动脉粥样硬化斑块特点的相关性。方法 收集198例原发性高血压患者,对其进行256层螺旋CT颈动脉血管造影检查。依据血浆同型半胱氨酸(homocysteine, Hcy)水平将其分为H型及非H型高血压组,比较两组患者颈动脉斑块特点及斑块造成的颈动脉狭窄程度的差异。再按照血压水平分层,分别比较两组上述的差异。结果 颈动脉总斑块在两组间的构成(OR=1.931,95%CI:1.035~3.601,χ2=4.281,P=0.039)、颈动脉不稳定斑块在两组间的构成(OR=2.780,95%CI:1.631~4.738,χ2=14.123,P=0.000)及颈动脉中度及以上狭窄在两组间的构成(OR=2.487,95%CI:1.398~4.424,χ2=9.613,P=0.002)差异均有统计学意义,而颈动脉稳定斑块在两组间的构成(OR=1.622,χ2=2.731,P=0.098)差异无统计学意义;按血压水平分层,颈动脉总斑块在两组间的构成(ORMH=2. 137,95%CI:1.723~2.651,χ2=47.767,P=0.000),颈动脉不稳定斑块在两组间的构成(ORMH=2.971,95%CI:1.649~5.354,χ2=13.135,P=0.000)及颈动脉中度及以上狭窄在两组间的构成(ORMH=2.584,95%CI:1.536~4.347,χ2=12.803,P=0.000)差异仍有统计学意义。结论 颈动脉斑块的发生、斑块的不稳定性及斑块造成的颈动脉狭窄程度均与H型高血压有关,且不受血压水平的影响。

Correlated Study on H-type Hypertension and Characteristics of the Carotid Arteriosclerosis Plaques
Abstract:Objective To study the correlation between H-type hypertension and characteristics of the carotid arteriosclerosis plaques. Methods We selected 198 inpatients with primary hypertension. All patients underwent 256-slice spiral CT carotid angiography. According to the level of Homocysteine,the subjects were divided into H-type and Non-H-type hypertension group. The characteristics of the carotid arteriosclerosis plaques and the level of the Carotid stenosis were compared between two groups.Then stratified according to blood pressure levels, the difference between the two groups was compared. Results It showed statistically significant difference in the total plaques composition between the two groups(OR=1.931, 95%CI:1.035-3.601,χ2=4.281,P=0.039). So was the unstable plaques composition(OR=2.780, 95%CI:1.631-4.738,χ2=14.123,P=0.000)and the level of the carotid stenosis composition(OR=2.487,95%CI:1.398-4.424,χ2=9.613,P=0.002). A significant difference was not found in the stable plaques composition between the two groups(OR=1.622,χ2=2.731,P=0.098). Stratified according to blood pressure levels, it still showed statistically significant difference in the total plaques composition between the two groups (orMH=2. 137,95%CI:1.723-2.651,χ2=47.767,P=0.000). So was the unstable plaques composition (ORMH=2.971,95%CI:1.649-5.354,χ2=13.135,P=0.000) and the level of the carotid stenosis composition (ORMH=2.584,95%CI:1.536-4.347,χ2=12.803,P=0.000). Conclusion The characteristics of the carotid arteriosclerosis plaques and the level of the Carotid stenosis were associated with H-type hypertension, and it not be effected by the blood pressure levels.
