食物不良反应患者血清中20种食物特异性IgG、IgG4的检测分析

中文摘要:目的 探讨食物特异性IgG(sIgG)和特异性IgG4(sIgG)检测在筛查食物不良反应患者问题食物的应用和临床意义。方法 采用酶联免疫吸附法(ELISA)对284例食物不良反应患者血清中20种食物sIgG抗体和sIgG4抗体进行平行检测。结果 食物sIgG总体阳性率55.8%,sIgG4总体阳性率44.0%,sIgG总体阳性检出率极显著高于sIgG4(P<0.01)。食物不良反应患者的sIgG和sIgG4的阳性率均明显高于健康对照组的sIgG和sIgG4(P<0.05)。小麦、大豆、花生、牛奶、鸡蛋、鳕鱼、榛子、腰果的sIgG和sIgG4检测结果的一致性较好。20种项目中,除蘑菇、花生外,其余18项食物的sIgG和sIgG4阳性率性别间差异均无统计学意义(P>0.05)。儿童组小麦、大豆、西红柿、牛奶、鸡蛋、鳕鱼、榛子、腰果和桃的sIgG阳性率均极显著高于青少年及成人组(P<0.05),小麦、鸡蛋、牛奶、鳕鱼、腰果的sIgG4阳性率均极显著高于青少年及成人组(P<0.05)。结论 检测食物不良反应患者尤其是儿童血清中食物sIgG和sIgG4对于指导剔除问题食物,缓解食物不良反应症状具有较大的临床意义。

Study on Specific IgG and Specific IgG4 of 20 Kinds of Food in Serum of Patients of Adverse Food Reactions
Abstract:Objective To investigate the clinical application and clinical significance of examination of food specific IgG and specific IgG4 to screen the food resulting adverse food reactions. Methods SIgG and sIgG4 to 20 food antigens were measured by ELISA in serum of 284 patients of adverse food reactions. Results The total positive rates of food sIgG and sIgG4 were 55.8% and 44.0% respectively. The total detection rates of food sIgG were significantly higher than sIgG4. The detection rates of 20 food sIgG and sIgG4 in patients of adverse food reactions were all significantly higher than that in healthy people(P<0.05). The detection results of sIgG to wheat, soybean, peanut, milk, egg, codfish, hazelnut, cashewnut had good consistencies with sIgG. Except mushroom and peanut, there was no significant difference between the detection rates of sIgG, sIgG and sIgG4 to the other 18 foods(P>0.05). The positive rates of sIgG to wheat, soybean, tomato, milk, egg, codfish, hazelnut, cashewnut and peach in children were distinctly higher than that in elder children and alduts(P<0.05). The positive rates of sIgG4 to wheat, egg, milk, codfish and cashewnut in children were distinctly higher than that in elder children and alduts(P<0.05). Conclusion Detection of food sIgG and sIgG4 had significant clinical signification as a guidance to eliminate the harmful food and improve the symptoms of adverse food reactions.
