577nm微脉冲激光联合七叶皂苷钠治疗糖尿病性黄斑水肿的疗效观察

中文摘要:目的 观察577nm微脉冲激光联合七叶皂苷钠治疗糖尿病性黄斑水肿的疗效。方法 2012年2月~2016年1月选择在笔者医院诊治的糖尿病性黄斑水肿患者144例(144眼)作为研究对象,采用简单随机、平行对照法分为观察组与对照组各72例,对照组选择577nm微脉冲激光治疗,观察组在对照组治疗的基础上联合七叶皂苷钠治疗,都治疗观察1个月。结果 治疗后观察组与对照组的有效率分别为97.2%和84.7%,观察组明显高于对照组,两组疗效比较差异有统计学意义(P<0.05)。观察组与对照组治疗后的最佳矫正视力分别为1.10±0.24和0.81±0.18,都明显高于治疗前的0.59±0.11和0.58±0.17(P<0.05),同时观察组治疗后的最佳矫正视力也明显高于对照组(P<0.05)。治疗期间观察组的荧光渗漏发生率明显少于对照组(P<0.05),两组前房炎性反应、角膜水肿、高眼压等并发症发生情况比较,差异无统计学意义(P>0.05)。结论 577nm微脉冲激光联合七叶皂苷钠治疗糖尿病性黄斑水肿提高视力,减少荧光渗漏的发生,具有很好的安全性与有效性,值得在临床上推广应用。

Efficacy of 577nm Micro Pulse Laser Combined with Seven Sodium Saponin in theTtreatment of Diabetic Macular Edema
Abstract:Objective To observe the efficacy of 577nm micro pulse laser combined with seven sodium saponin in the treatment of diabetic macular edema. Methods From February 2012 to January 2016, 144 patients (144 eyes) with diabetic macular edema in our hospital for diagnosis and treatment were selected as the research object, used simple randomized, parallel controlled method, all the patients were equally divided into observation group and control group, the control group was given the 577nm micro pulse laser in the treatment, the observation group was given the micro pulse laser combined with seven sodium saponin in the treatment, observed the prognosis in the two groups. Results The effective rate of the observation group and the control group were 97.2% and 84.7% respectively that the observation group was significantly higher than the control group, the difference was statistically significant (P<0.05). The best corrected visual acuity after treatment in the observation group and control group were 1.10±0.24 and 0.81±0.18 that were significantly higher than before treatment of 0.59±0.11 and 0.58±0.17 (P< 0.05), and the best corrected visual acuity after treatment in the observation group were significantly higher than those in control group (P<0.05). During the treatment period, the incidences of fluorescence leakage in the observation group was significantly less than that in the control group (P<0.05), and there were no significant difference in the incidences of inflammatory reaction, corneal edema and high intraocular pressure compared between the two groups (P>0.05). Conclusion 577nm micro pulse laser combined with seven sodium saponin in the treatment of diabetic macular edema can improve vision, reduce the occurrence of fluorescence leakage, it has good safety and effectiveness that is worth to be popularized in clinical application.
