中文摘要:目的 探讨IL-17、IL-23在前列腺增生组织中的表达及临床意义。方法 收集80例经尿道前列腺电切术(transurethral resection of prostate,TURP)的良性前列腺增生(benign prostatic hyperplasia,BPH)患者的术中前列腺组织标本,根据组织病理学特征其是否合并组织学炎症(histological prostatitis,HP)分为单纯组和炎症组。意外死亡年轻男性经法医尸检5例标本为对照组。术前常规行血PSA、经直肠B超检查及IPSS评分等。采用免疫组化法检测IL-17和IL-23在前列腺组织中的表达情况。结果 80例TURP切除的前列腺组织标本中,有60例(75%)患者前列腺组织中存在组织学炎症。炎症组的前列腺体积及血清PSA显著大于单纯性BPH组,最大尿流率也显著低于单纯性BPH组(P<0.05)。炎症组的IL-17、IL-23含量明显高于对照组和单纯组(P<0.05),尤其在合并中重度组织学炎症的BPH标本中。随着前列腺组织学炎症分级的升高,IL-17和IL-23的表达率也升高,IL-17、IL-23在伴有组织炎症的前列腺增生组织的表达呈正相关(r=0.502,P<0.05)。结论 IL-17、IL-23与BPH组织炎症呈正相关,提示组织学炎症可能与前列腺组织学增生及临床进展过程相关。

Expression and Significance of IL-17,IL-23 in Benign Prostatic Hyperplasia
Abstract:Objective To study the expression and clinical significance of IL-17 and IL-23 in benign prostatic hyperplasia. Methods According to the preoperative findings of transrectal prostate ultrasonography,serum prostate specific antigen (PSA),international prostate symptom score (IPSS) and postoperative histopathology,80 cases of BPH pathologically confirmed after transurethral resection of the prostate (TURP) were divided into a BPH group (n=20) and a BPH + HP(combined histological prostatitis) group (n=60).Another prostate tissue from healthy male 18-27 years old died of trauma accidental death forensic autopsy is control group(n=5). The expressions of IL-17 and IL-23 in the prostate tissue were determined by immunohistochemistry. Results Of the 80 cases,60 were found to be BPH+HP,and the other 20 to be simple BPH. The volume of the prostate,serum PSA in BPH+HP was obviously larger, and maximum urine flow rate was lower than that in simple BPH (P<0.05). The expressions of IL-17 and IL-23 were significantly higher in the BPH+HP than in the BPH group and control group (P<0.05). The expression of IL-17 and IL-23 in associated with BPH+HP and BPH was positively correlated(r=0.502, P<0.05). Conclusion The unregulated expressions IL-17 and IL-23 may suggest a certain association between HP and BPH, and that inflammation may be a pathogenic factor of BPH and correlate with its pathological and clinical development.
